Care Management Associates, a division of Connecticut Community Care, Inc., helps elders and adults with disabilities find the resources needed to live as independently as possible. We provide comprehensive geriatric assessments and case management. Our care managers are nurses and social workers with years of experience in care management. We are members of the National Association of Professional Geriatric Care Managers. In 2007 and 2009, we were chosen by Connecticut's attorneys and law firms as "The Best" in Home Care through a survey done by the Connecticut Law Tribune.
